Midwest DCE is a Medicare REACH ACO – Realizing Equity, Access, and Community Health Accountable Care Organization. Midwest is one of a limited number of organizations across the country participating in the model with the Centers for Medicare and Medicaid Innovation (CMS). If your physician is participating with Midwest DCE, you will receive a letter in the mail welcoming you.

ACO Anonymous Compliance Reporting

Midwest has an Anonymous Compliance Reporting webform that is accessible to all individuals including Medicare Beneficiaries. This form will allow you to report any compliance concerns related to the ACO, and can be accessed by visiting: pearlhealth.com/anonymousreporting. You can choose to report anonymously or provide your contact information. The information you provide will be transmitted to the ACO Compliance Officer who will follow-up on the report. The ACO has a non-retaliation policy which means you don’t need to fear making a good faith report. We rely on individuals like you to let us know when something is wrong.
